Baptismal Certificate & Family Identification Number
Submit a baptismal certificate for each child (including those baptized at Saint Andrew). Provide full dates and church names (with copies, if applicable) for Penance, Eucharist, and Confirmation.
Family IDs are found on your weekly parish donations envelope. If you are not currently a member of Saint Andrew Parish, the name of your current church affiliation is required.
Students seeking admission to Saint Andrew Catholic School who are not currently baptized or affiliated with a Catholic parish are warmly welcomed into our community. While baptism is not required for acceptance, families should understand that Catholic identity is central to the school's life. All students participate in religious instruction, prayer, and liturgical celebrations as part of their formation.
As part of the admissions process, families of unbaptized students will be asked to provide a brief written statement confirming that their child has not been baptized. This helps us ensure accurate sacramental records and provide appropriate pastoral support.
For families interested in baptism or full initiation into the Catholic Church, we are happy to guide you through the process and connect you with parish resources. Our goal is to accompany each student and family with respect, clarity, and a spirit of invitation as they explore the richness of the Catholic faith.
Provide all testing results (psychological, physical, educational, disability, etc.), along with school records, recent report cards, and standardized test results at registration.
Include a copy of your child’s current immunization record and the parent-completed health history form with your registration packet. The record must be signed by a doctor.
Registrations without these documents will not be accepted.
