Financial Assistance

Submit the documentation in support of, and to complete your application.

Any documents received are imaged, and then immediately and securely destroyed.

Federal Tax Information

• Your most recent IRS Federal Form 1040/1040A/1040-EZ

• If parents file separately, you must submit both returns

• State tax returns are not required

If applicable, include the following:

• Business income: Schedule C or Form 4562
• Farm income: Schedule F + Form 4562
• Rental property: Schedule E (page 1)
• S-Corporation: Schedule E (page 2), Form 1120S (five pages), Schedule K-1 (under schedules, halfway down page), Form 8825
• Partnership: Schedule E (page 2), Form 1065 (five pages), Schedule K-1 (under schedules, halfway down page), Form 8825
• Estates/Trusts: Schedule E (page 2), Form 1041, Schedule K-1 (under schedules, halfway down page)

If you are self-employed with do not have a W-2, you must submit your current Federal Tax Return.

Non-Taxable Income Documentation

• Social Security, Child Support, Food Stamps, Workers' Compensation, TANF, etc.
• If you do not file taxes, you must provide documentation for ALL income received

Complete the online application at any time and upload or fax all required supporting documents.

• PDFs only, each file under 20MB
• If faxing, send to (866) 315-9264

Please do not use cell phone photos of supporting documents as they often cannot be read in the system.

You can log in at any time to review your application status. Please allow two (2) weeks for initial processing after ALL documents are received.

A non-refundable $30 fee may be required before submission.

• The school sets scholarship deadlines—if applying after a deadline, please confirm acceptance with the school
